Montgomery County Will Offer Free Virtual Training on ‘Chronic Care Model’ on Thursday, June 8, for Health Care Professionals and Medical Administrators

Health care providers in Montgomery County are invited to attend a free, virtual training on the Chronic Care Model (CCM) from noon-1:30 p.m. on Thursday, June 8. Sponsored by the Public Health Services of the County’s Department of Health and Human Services, the training is supported through the Community Health Resource Commission grant under the State of Maryland and will be facilitated by Mike Hindmarsh from The Centre for Collaboration, Motivation and Innovation.

Insights from Huixing (Kate) Lu, who is with the Chinese Culture and Community Service Center, Inc., will be presented during the last 30 minutes of the training.

The CCM identifies the essential elements of a health care system that encourage high-quality chronic disease care. These elements are the community, the health system, self-management support, delivery systems design, decision support, and clinical information systems. CCM can be applied to a variety of chronic illnesses, health care settings and target populations.

The training is best suited for hospital and clinic administrators, physicians, registered nurses, physician assistants, certified medical assistants and other care providers who might benefit from learning more about the CCM and benefits of implementing it in their practice.
